Intake form for coaches: what do you ask?
A good intake saves you the first hour and gives your client a calm start. This is what belongs in it, and what you had better leave out.
What belongs in it
- Contact details. Name, email address and phone number.
- The request for help. What does the client want to achieve, and why now?
- Context. Work, situation and earlier guidance, only as far as relevant.
- Practical arrangements. Availability and preference for a conversation on location or online.
- Consent. Agreement to the processing of data and your way of working.
What you had better leave out
Only ask what you really need. The more sensitive data you store, the heavier the duty of care. For example, do not routinely ask about health if it is not needed for your guidance.
